Insight Global is looking for an Inpatient Clinical Social Worker to join their hospital for in Worcester, MA!

Day To Day:

This individual will be working in the Department of Mental Health inpatient units under a Department of Public Health, a 370 bed accredited hospital.
The caseload includes patients admitted for short term, sub-acute medical stabilization, reconditioning, physical rehab as well as some longer term patients, some with SORB status.
The caseload includes a heavy con centration of patients with numerous and complex psycho-social components that impact on medical care and treatment as well as discharge planning: poly-substance abuse/addictions: homelessness and poverty; chronic neuro-psychiatric/neuro-degenerative illnesses; involvement with the court system; positive CORI/SORI status and marginal/impoverished support systems.
The primary responsibilities for this position are case management and discharge planning services, with other responsibilities including: patient/family education, crisis intervention, high risk screening, psychosocial assessments, interdisciplinary team planning and documentation in the Electronic Medical Record.
Case load starts at 4-5, incrementally increases to 10-12.
