Job Responsibilities:
- Assess the social and emotional factors related to the clients health status
- Develop and implement a social service care plan
- Collaborate with other healthcare professionals to ensure quality care
- Maintain accurate and timely documentation of services
- Provide counseling and emotional support to patients and families.
- Facilitate support groups and educational sessions.
- Connect patients with community resources and support services.
- Collaborate with external agencies and organizations.
- Anticipate in care conferences and collaborate with the healthcare team.
- Maintain accurate and timely documentation of patient interactions.
- Advocate for patients' rights.
Minimum Requirements:
- Licensed Medical Social Worker in the state of Florida Required
- Master's Of Social Work (MSW) Degree Required
- Basic Life Support (BLS) Required
- Valid Driver's License Required
- One Year of Home Health Experience Required
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of medical terminology and healthcare regulations
- Ability to work with a diverse population
- Creole or Spanish Speaking Preferred
