Responsibilities:
- Conduct comprehensive psychosocial assessments of patients and families.
- Develop and implement individualized discharge plans in collaboration with the care team.
- Coordinate referrals to appropriate community resources and post-acute care services.
- Facilitate placements into skilled nursing facilities (SNFs), long-term acute care (LTAC), assisted living, and guardianship arrangements.
- Advocate for patient rights and access to necessary services.
- Work collaboratively with physicians, nurses, case managers, and multidisciplinary hospital staff.
- Provide crisis intervention and counseling for patients and families as needed.
- Maintain accurate and timely documentation in the electronic medical record system.
- Participate in interdisciplinary rounds and team meetings.
- Adhere to all hospital policies, procedures, and applicable regulatory requirements.
Must-Have Skills:
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution.
- Minimum of 1–2 years recent inpatient hospital experience.
- Demonstrated expertise in discharge planning and community resource navigation.
- Experience with placement processes for SNF, LTAC, assisted living, and guardianship cases.
- Strong communication, organizational, and problem-solving abilities.
- Ability to work independently and collaboratively in a fast-paced hospital setting.
Nice-to-Have Skills:
- Active Licensed Social Worker (LMSW, LCSW, or equivalent) in the state of Maryland.
- Experience providing crisis intervention and counseling in hospital settings.
- Familiarity with electronic medical record documentation processes.
- Knowledge of hospital regulatory requirements and best practices.
- Participation in interdisciplinary rounds or team meetings in acute care environments.
