Job Description

Job Summary:

The Healthcare Referral Coordinator is responsible for managing and coordinating patient referrals to ensure efficient and timely access to necessary medical services. This role involves interacting with healthcare providers, insurance companies, and patients to facilitate smooth transitions of care. The ideal candidate will possess excellent communication skills, a strong understanding of healthcare systems, and the ability to multitask in a fast-paced environment.

Key Responsibilities:

Referral Management:

  • Receive, review, and process referral requests from primary care providers, specialists, and other healthcare professionals.
  • Ensure all necessary documentation and patient information is complete and accurate before forwarding referrals to appropriate services.
  • Track and monitor the status of referrals to ensure timely processing and follow-up.

Patient Coordination:

  • Contact patients to inform them of referral details, including appointment times, locations, and required preparations.
  • Provide patients with information regarding their referral, including what to expect during their appointment and any follow-up instructions.
  • Address patient inquiries and concerns related to referrals, providing clear and compassionate communication.

Insurance and Authorization:

  • Verify patient insurance coverage and obtain necessary authorizations for referred services.
  • Collaborate with insurance companies to resolve any issues or delays in authorization.
  • Maintain up-to-date knowledge of insurance policies and procedures to ensure compliance and efficient processing.

Communication and Collaboration:

  • Serve as a liaison between patients, healthcare providers, and insurance companies to facilitate effective communication and coordination.
  • Work closely with healthcare providers to ensure accurate and timely referral information is shared.
  • Coordinate with various departments and specialists to schedule patient appointments and procedures.

Documentation and Record Keeping:

  • Maintain accurate and detailed records of all referral activities and patient interactions.
  • Ensure all referral documentation is filed and stored in compliance with HIPAA and organizational policies.
  • Generate and analyze reports related to referral activities to identify trends and areas for improvement.

Qualifications:

  • Education and Experience:
  • High school diploma or equivalent required; associate or bachelor's degree in healthcare administration or related field preferred.
  • Previous experience in a healthcare setting, particularly in referral coordination or patient services, is highly desirable.
  • Familiarity health records (EHR) systems. EPIC preferred

Skills and Abilities:

  • Excellent verbal and written communication skills.
  • Strong organizational skills and attention to detail.
  • Ability to multitask and prioritize in a fast-paced environment.
  • Proficiency in Microsoft Office Suite and experience with EHR systems.
