Join us as RN Case Manager, – Make an Impact!

Location: Bronx, NY 10461

Duration: 9 Weeks (Contract) + Extension

Shift: 8 hrs. Days, 9:00 am 5:00 pm, (M-F)

Scheduled Hours: 40 Hours a Week .

Hourly Pay: $56.00 – $64.00

Job Duties:

• Review patient records to assess medical necessity and appropriate level of care.
• Coordinate care plans and discharge planning with providers and patients.
• Ensure services meet insurance and regulatory guidelines.
• Collaborate with physicians, nurses, and insurance companies.
• Monitor patient progress and resource use.
• Document utilization reviews and maintain compliance with policies.

Minimum Qualification:

• Must have an Active RN License issued by NYSED
• At least two years of experience.
