- Preferably has 2 years of hospital experience
Job Description
Under the direction of the Case Management Director and with minimal supervision, the Inpatient Social Worker/Patient Counselor is responsible for:
- Assisting patients with bio-psychosocial needs
- Facilitating access to medical care and community resources
- Supporting patients in achieving their highest level of health
Key Responsibilities:
- Provide crisis intervention and short-term counseling for patients and families
- Coordinate discharge planning in collaboration with the care team, patient, and family
- Support patient transitions along the continuum of care
- Connect patients to both internal services and external community resources
Required Qualifications
- Master’s degree in social work or counseling psychology
- Minimum of 1 year of experience in a social work or counseling role
Preferred Qualification
- At least 2 years of experience in a hospital or inpatient setting
